Enrollment Paperwork
Below is the paperwork required to enroll a new member into the Platte Canyon Club. We need one family form for each family, and a member form for each child who will be coming to Boys & Girls Club. Completed paperwork can be emailed to plattecanyon@bgchr.co or sent to the elementary school to be put into the Boys & Girls Club mailbox.
